HMR Integrated Community Diabetes Service
We are the Bury and Heywood, Middleton and Rochdale (HMR) Diabetes Service.
We are a team of community and hospital diabetes staff who work together in one single service - to ensure people's care is joined up.
Who is the service for?
The service is for adults, aged 18 years and above, who have diabetes and are registered with a GP in Bury or the Rochdale borough.
What does the service provide?
We provide the majority of your diabetes care within the local community, Depending on your needs and the complexity of your condition, you could be cared for by the Bury and HMR Diabetes Service directly, or even by your GP or practice nurse.
Only people needing the most complex and specialist services will now receive their care at Fairfield General Hospital or Rochdale Infirmary for example pregnant, Active Foot, and Diabetes Pump patients Each referral will be triage and allocated to the most appropriate clinic.
We also provide training and education for GPs and primary care clinicians to support them to effectively manage your condition and reduce the risks of any further complications.
We will also provide you with support to manage your condition through a nationally accredited programme.
You need to be referred to the service via your GP.
The service will benefit you in many ways, including:
- A better experience overall
- Better quality of care
- Improved co-ordination of your care
- Reduced risk of needing to be admitted to hospital
- Care closer to home for many people
- Greater support to manage your condition
- Faster access to any specialist care you may need
There are also many benefits to the NHS, for example improved partnership working and being more cost effective. We work as part of a township team, but we provide care across the borough.
