Important information regarding the provision of community healthcare services
From Monday 1st July 2019, the following community healthcare services will be provided in your area by Salford Royal NHS Foundation Trust as part of the Northern Care Alliance NHS Group (NCA) and its local integrated health teams:
- Bury Community Services
- Heywood, Middleton and Rochdale Adult Therapy Services
- Oldham Community Services.
What this means for you
These services are still provided by the NHS but run by a different NHS Trust.
NHS staff are working closely together to ensure a smooth transfer of services.
There will be no change or impact to the service or treatment you receive; the existing staff working within these services will continue to provide your care.
If you have an appointment booked, you should still attend, unless you hear otherwise from the service. Should you need to contact the service, you can do so using the current contact details.
Following the 1st July, you will start to see a different NHS organisation logo on patient letters, leaflets and other materials you may receive.
Transferring your Information
If you are currently receiving care from one of these services and it will continue beyond 1st July 2019, your information and care record will remain with the service.
Any transfer of information will be done securely, handled confidentially and in accordance with Data Protection Legislation.
