Diabetes Oldham
Who is the service for?
Community outpatient services for people with diabetes related complications, diagnosis of diabetes and on going support/treatment for complex diabetes care such as co-morbidities, feet and dietetics input. The service accepts GP and Practice nurse referrals for outpatient care, GP referrals and emergency support via A&E for diabetic complications.
What does the service provide?
The service provides one to one support with a Consultant Diabetologists, Diabetes Specialist Nurse, podiatrists or dietetics, there is also an established multi disciplinary team for complex.
The service provides the following appointment types:-
- MDT
- Consultant
- DSN
- Podiatry
- Dietetics
- Domiciliary Care
The services also provide structured education programmes for both the public and primary care.

Monday – Friday 9am to 5pm.
