Bury Specialist Palliative Care

The Bury Community  Palliative Care Nursing Team, as members of the Bury Specialist Palliative Care Multi-Disciplinary Team (MDT), work alongside, and in support of, statutory and voluntary health and social care services to improve the care of patients and their families with complex palliative needs.

Care is provided to adults from diagnosis to end of life, where the patient's needs are not being met by the generic Primary Health Care Team (PHCT). 

Care is offered to patients in their own homes, Nursing or Residential Homes.

The service aims are:

To work with PHCT members to help meet the physical, psychological, spiritual and social needs of adult patients and carers experiencing difficulties managing symptoms or adjusting to life with a diagnosis of cancer or other life limiting illnesses.

To help to facilitate continuity of care across all health care settings so as to enable patients to be cared for in their preferred place of care.

To work with other members of the specialist palliative care MDT to ensure that the best possible evidence-based care is given to patients and carers.
