Like our patients and their families and friends, we want to make sure our patients are feeling well and strong enough to go home as soon as possible, either in the care of responsible adults they trust or healthcare professionals working in the community, who can visit them in their homes or a local clinic.
Once staff are satisfied that it is appropriate to allow a patient to go home, they will follow Trust wide procedures on their discharge.
Before you are discharged home, your relatives or carers will be asked to bring your clothes into hospital for you. You will also need to arrange transport with family or a friend, or book a taxi. Ambulances will only be arranged if you have a clinical need for one. You will need to make sure you can get into your home and should also ask any questions you have about your care after discharge.
When you are ready to go home, a doctor or nurse will discuss your discharge arrangements with you. You will be given:
- A supply of any medication you may need
- A letter informing you of any referrals, appointments or tests which have been arranged for you
- A discharge letter (a copy will also be sent to your GP)
- If you require a district nurse then this will be arranged for you before you are discharged home.
Once you are ready to go home, you may be transferred to the discharge lounge. This is a place where you wait to be collected by your carers, relatives or by an ambulance. Ward staff will arrange for any valuables they have been asked to look after to be returned to you, and that you have all your personal property with you to take home. The ward staff will hand you your discharge letter and talk to you about any drugs, medications and dressings you need. They will also make sure any community support you require is in place prior to your discharge and provide contact details for somebody that you can contact if you have any questions following your discharge.
If you are prescribed medication to take following discharge you will be given one weeks supply. If and further medications / dressings / medical supplies are required, you will need to contact your GP who will prescribe these for you. Information about how and when to take your medication is on the bottles or packets supplied. Leaflets explaining any side effects will also be provided. If you or a member of your family have any questions or concerns regarding your medications, please speak to a member of the ward staff.
Fit notes / claim forms
Please ask a member of the ward staff about this; they will be able to arrange for fit notes and claim forms to be signed. You may be charged for some claim forms, but the ward will be able to provide you with this information.
If you require a follow up appointment this will either be arranged before you leave the ward, or you will receive an appointment in the post.
All wards and outpatient areas have comment cards and boxes in which to place the completed cards. These boxes are emptied by volunteers each week and ward staff discuss the comments cards. The information patients and families provide is used to improve our services, helping to create a high quality patient experience which you will be happy to recommend to others.
Friends & Family Test
