Who is the service for?
The unit provides 24 community beds. It can be accessed by any adult aged 18 years and over who resides or is registered with a GP in the Heywood , Middleton and Rochdale area.
What does the service provide?
The service is part of a wider intermediate care and reablement service. The unit provides intermediate care which is enhanced by nursing and therapy provision and which supports people who are medically stable to return to their homes.
Who will patients meet while under your department’s/service’s care?
The unit is staffed with; Registered nurses, Physiotherapists (who will work closely with service users to identify realistic and functional goals to promote independence), Occupational therapists (who will work with service users to ensure the discharge home to a safe environment is achieved with aids and adaptations as required), Assistant practitioners (who work closely with service users under the supervision of the therapy team) and a dedicated team of care support staff. Medical cover provided by general practitioners and social work teams work within the unit to enable assessment and access to ongoing care packages. In addition support with medicines is provided by pharmacists and technicians. The unit also supports people through a care connector service provided by Age UK Rochdale, The Big Life group and health promoters.

The service is available 365 days a year 24 hours a day.
Where is the service provided?
The service is provided at Rochdale infirmary on a purpose built unit- each bed is in a single room with 15 en-suites available. Television facilities are available in each room.
There is a lounge /dining room and quiet lounge with lots of sitting areas available throughout the unit.The unit has an active and varied programme of events designed with service users to enhance the stay.
