Who is the service for?
Your health care practitioner can refer you to the Nutrition Support team if you are having difficulties eating enough to maintain your weight.
What does the service provide?
The Dietitians and Assistant in the nutrition support team can provide information and practical suggestions to help you to stop losing weight and move towards health. For example they can help if you have a sore mouth or swallowing difficulties or a poor appetite.
Who will patients meet?
The Dietitians and Assistant will take into account your diagnosis and your choices to agree a plan with you. They will also review the effect of the plan with you and agree changes if necessary.
When is the service available?
The Nutrition Support Team works 8.30am - 4.30pm, Monday – Friday.
Where is the service provided?
The Nutrition Support Team provide out patient clinics at Croft Shifa, Rochdale, Phoenix Centre, Heywood and Durnford St Health Centre, Middleton. They also visit patients in their own homes who are housebound and attend nursing and care home residences.
